Club & Executive Information Reporting Form – Due as soon as your Club Exec elections are complete
Each year, it is very important that clubs submit the names of their Club Executive and provide club contact information.
Clubs can provide this information using the Club & Executive Information Reporting Form.
Please designate one person from your club to submit the form once you have completed your club elections or determined that current executives will be staying on for another year.
IMPORTANT: Even if your club’s leadership and contact information is staying the same as last year, the form is still required. This allows HQ to double check that all information in our database is correct as well as update the year on your Club Executive members’ files.
